CARE GIVING TEAM
Certifies the patient has a terminal illness and remains the patient's primary physician and works as a member of the hospice team.
Certifies the attending physician's prognosis; leads the hospice team in developing a plan care; provides consultation to the attending physician.
Evaluates patient's condition during regular scheduled visits. The registered nurse coordinates the plan of care and teaches the family proper patient care techniques; the use of medication and management.
Assess social needs of the patient's, family and provides psychological and Bereavement counseling.
Provides spiritual support. Works as a liaison with clergy of the patient's choice.
